A. Information About Participants in Clinical Trials

For any questions relating to your participation in a clinical trial, contact the Principal Investigator on the study and/or refer to the Informed Consent and other trial documents provided to you.

B. Information About Patients

If you contact Amgen regarding your experience in participating in one of our patient support programs or using one of our products or services, including to report an adverse event ('side effect') or a defect or quality issue with the Amgen product you received, we may ask you for specific facts, including patient initials, age, gender, and the prescribing physician. We may use the information you provide in submitting reports to regulatory agencies as required by law (e.g., the Central Drugs Standard Control Organization, the U.S. Federal Food and Drug Administration; the European Medicines Agency) and/or for quality or support purposes relating to the administered programs. We also may use the information to contact the prescribing physician to follow up regarding an adverse event ('side effect') involving use of our product.

We may disclose your personal information to third parties performing work on our behalf, such as our call centre operators and other entities that assist us in responding to and processing your inquiries and product experience reports but only after taking reasonable steps to ensure that we can share such information in compliance with applicable privacy laws.

C. Information About Healthcare Professionals and their Staff

If you are a Healthcare Professional, we may collect information about you when you interact with us, participate in or request to be considered for Amgen-related activities, such as clinical trials, advisory boards, or consultancies, register for an Amgen-sponsored event (including online events), or contact us for medical information, safety or quality issues. In addition, we may collect professional information from Healthcare Professionals from other sources, such as from our service providers. Amgen takes steps to ensure that the information it receives has been collected and transferred in compliance with applicable privacy laws.

The personal information we process about Healthcare Professionals generally includes name, contact information, login/password for Amgen services online, curriculum vitae and professional information such as areas of specialization, past collaborations with Amgen, dates and brief summary of interactions with Amgen's sales representatives or medical staff, and financial and banking information, if necessary to pay you for services or reimburse your expenses. We use this personal information to manage our commercial and scientific activities, promote and assess our products and services, respond to requests for medical information, assist Healthcare Professionals in planning trips related to Amgen's business or products, comply with legal and regulatory requirements, identify and engage with Healthcare Professionals as scientific experts or professional leaders, and for purposes of storage and backup. We may disclose Healthcare Professional personal information to regulatory authorities and ethics committees, our data processors (for example, a travel service or clinical research organization), and third parties (for example, other pharmaceutical companies with which Amgen collaborates), but only after taking reasonable steps to ensure that we can share such information in compliance with applicable privacy laws.

D. Information About Users of Our Website

Personal information as used in the context of this statement, applies to any information that identifies or that can be used to identify you, such as your name, address, telephone number, and e-mail address.

We retain a record of the IP address of your computer. We will only be able to link a particular IP address to you personally if you have provided us personally identifying information whilst using our website. F. Information About Other Individuals

If you are a Healthcare Professional or a healthcare organization, it is your responsibility to obtain your patient's authorization and consent to provide his or her health-related personal information to Amgen through our Web sites. This also applies when you are entering personal information on behalf of someone; i.e. if you are a caregiver. By submitting any such information, you represent and warrant that you have obtained the patient's authorization to do so. Please note that patient information may not be disclosed on an Amgen discussion forum or chat room.

G. Legal Bases for Processing

Amgen processes your personal information in accordance with applicable law and only where we have obtained your consent or where we are permitted by applicable law to conduct such processing for certain legitimate uses. We have relationships with different vendors and suppliers who help us design and maintain our computer systems and security, respond to e-mail and customer inquiries, analyse our data, create special promotions, and otherwise support our business operations.
